Offset handlebar clamps

Does anyone know where I can buy the type of top clamp that moves the handlebars back a little and up slightly. Something like 1 1/2" back and 1/2"-1" up .I don't remember exactly .I had some on my last bike and it certainly helped .Thanks in advance.
